A. All Employees must follow HIPAA standards

B. Medical Information refers to past, present, or future information regarding the client’s personal medical documentation, including payment information.

C. BTB holds our ‘Privacy Policy’, Compliance to HIPAA, and personal trust to the highest legal and ethical standards. BTB and our employees with not knowingly do anything to harm you, your reputation, or your record in any way.

D. Ways your medical history may be shared: 1) Treatment - BTB will share files with outside providers in accordance with HIPAA and our policies set within as outlined in our informed consent. Documentation (Release or Exchange of Information Form) will be requested for signature when information does need to be shared, but not in all circumstances. For further details about limits to confidentiality or release of information, please ask your BTB provider or review a copy of your signed informed consent. 2) Payments - BTB may need to share some medical information, in accordance with HIPAA, to have third parties invoice, bill, file, and explain charges when needed. 3) Operations - BTB may use and disclose your medical information internally with business management, and for administrative actions, legal and auditing functions, and insurance-related activities. We may also disclose information to doctors, nurses, technicians, and other personnel for review and learning purposes, but will remove information disclosing your personal identity when possible. 4) Guardians and Caregivers: BTB may release information to guardians and consenting individuals above the age of 18 when required. 5) Notification - BTB may release the minimal information necessary about your location, well being, or other status as required for personal protection and health. 6) Contacts - We may contact you to provide appointment reminders, information about treatment or treatment alternatives, or other health related information that may be of interest to you. Standard text messaging rates and/or fees may apply. 7) Mental Health Information - State laws create specific requirements for the release of mental health records. We will obtain your specific authorization to release mental health or medical information when required by these laws.
